Please complete the following steps:
- Contact your Academic Advisor to review your research opportunity.

Once your Academic Advisor has reviewed your request and discussed all of the important details with you, your Advisor will provide you with the link to the CEOAS Experiential Learning Application Form.

You will complete the application form.

IMPORTANT: You will forward your application receipt email from Google Forms to your Research Mentor's work email address with instructions to review your application for final approval. Copy Fred Polinder at fred.polinder@oregonstate.edu to this email.
Suggested Text for Email:"Dear (name of Supervisor/Mentor), please review the details of my application in the email below that we've previously discussed. If you will approve, please forward this email indicating your approval to Fred Polinder at fred.polinder@oregonstate.edu or please provide feedback for any needed changes based on our previous discussions. Thank you."
- IMPORTANT: Your Research Mentor must reply to the email with written approval (or feedback for needed changes to edit) and copy Fred Polinder at fred.polinder@oregonstate.edu.
Final approval must be granted no later than Wednesday, Week 1 of the term you plan to register.
Approval cannot be granted retroactively for research completed previously.
